Plan Includes:

- All X-rays, 2 annual exams and 2 cleanings per year. (prophy or perio Maintenance )
- 20% off all Dental treatment procedures including SRP’s and problem focused-limited exams.
- New Patient exam not included. Cost is $235 in addition to membership cost. *Does not include product (whitening, take home fluoride etc.)*
- Fee Established Patient: $400 per patient
- New Patient Fee: $635 per patient

Additional family members under the age of 12 yrs old are $200. Plan must be renewed yearly on date of original purchase date to remain active, but renewal is optional.
